Research consistently shows that many people have partial tears or tendon wear on imaging without any symptoms at all. So a finding on a scan doesn’t automatically explain what you’re feeling, and it certainly doesn’t mean you need surgery.

Rotator cuff injuries can show up in different ways. Some people notice a sharp pain with specific movements like reaching behind their back or lifting away from the body. Others describe a deeper ache that builds through the day or wakes them up at night. In some cases, there’s noticeable weakness or a feeling that the shoulder just isn’t keeping up.

 The type of injury matters too. Rotator cuff tendinitis involves irritation of the tendon, often from repetitive use or a spike in activity. A tear, whether partial or full-thickness, involves structural damage to the tendon itself. And age-related tendon degeneration is a normal process that doesn’t always require intervention.

The important thing to know is that most rotator cuff problems, even many tears, can be managed effectively without surgery. Strengthening the muscles around the shoulder is often the most reliable path forward, and understanding where you’re starting from makes all the difference.

Common Causes of Rotator Cuff Injuries

Rotator cuff injuries can develop suddenly from a specific event, but more often they build gradually as the tendons are exposed to more demand than they’re currently prepared for. In many cases, it’s a combination of factors rather than a single cause.

Common contributors may include:

  • Repetitive overhead movements such as throwing, swimming, or painting

  • A sudden increase in shoulder activity after a period of reduced use

  • Age-related changes in tendon quality, which are a normal part of aging

  • Weakness or reduced endurance in the rotator cuff and shoulder blade stabilizers

  • Prolonged desk posture or sustained arm positions that load the shoulder

  • A fall onto an outstretched hand or direct impact to the shoulder

  • Stiffness through the upper back or neck that shifts demand onto the rotator cuff

  • Insufficient recovery between higher-load training sessions or physical work

Assessing a Rotator Cuff Injury

We start by talking through what’s been going on. When the pain started, whether there was a specific incident, what movements bother you most, and how it’s affecting your sleep, work, or training. Details like whether it hurts to reach overhead, behind your back, or out to the side all help narrow down which part of the cuff may be involved.

From there, we test how the shoulder is actually performing. This includes evaluating the strength of each rotator cuff muscle individually, assessing how the shoulder blade moves and stabilizes during loaded tasks, and looking at range of motion through the shoulder, neck, and upper back. Specific clinical tests can help us determine whether the issue is more likely tendon-related, a partial tear, or something else contributing to the pain.

We also look at the bigger picture. The rotator cuff doesn’t work in isolation. How the shoulder blade, upper back, and neck are functioning all influence how much demand is placed on the cuff tendons. If there’s stiffness or poor control in one of those areas, the rotator cuff often picks up the slack, and that’s frequently part of what drives the problem.

Imaging like ultrasound or MRI may be useful in certain situations, but it’s not always the first step. Many rotator cuff changes that show up on a scan are present in people with no pain at all, so what we find during a hands-on assessment often tells us more about what’s actually driving your symptoms.
